The first two documents are Microsoft form documents designed to be filled out electronically. To use the forms, the receiving computer must have Microsoft Word for Windows 97 or greater.  If you are using Microsoft Internet Explorer, just click the Word Form Document to view and use. If you are using Netscape or other Web browser, down load the files to your hard drive by moving the cursor over the Word Form Document and pushing the right mouse button. Chose [Save to Disk], then select where you want the file to be stored. If you can not use MS Word, select the PDF format to view and or print out the forms.
